 3.   Admission Process (ONLINE ADMISSION)

                                           (Walk-In) – Visit the Registrar’s Office located at the school left wing ground floor. Enrollment will be given for you to fill up.

Please get ready with your original and scanned (PDF/JPG file) documents to be submitted online depending on your entry status as follows:

For Senior High School Graduate

  1. Applicant Photo
  2. Certificate of Good Moral Character
  3. Senior High School Card
  4. PSA copy of Birth Certificate
  5. Copy of vaccine card

  For ALS Passer (SHS Level Certificate)

  1. Applicant Photo
  2. Certificate of Good Moral Character
  3. Certified True Copy of List of Passer
  4. Certificate of Rating with remarks equivalent to senior high school level/graduate and date of graduation
  5. PSA copy of Birth Certificate
  6. Copy of vaccine card

For Transferee 

  1. Applicant Photo
  2. Certificate of Good Moral Character
  3. Copy of Grades / Transcript of Record (Must have NO dropped and failing grade)
  4. Honorable Dismissal
  5. PSA copy of Birth Certificate
  6. Copy of vaccine card

For Elementary and Preschool

  1.   Applicant Photo
  2.  Certificate of Good Moral Character
  3. Copy of Grades / Transcript of Record (Must have NO dropped and failing grade)
  4. PSA copy of Birth Certificate

For FOREIGN STUDENTS

  1. Accomplish the Foreign Student Application Form
  2. Submit Copy of School Records (in English) for evaluation
  3. Submit Copy of Passport Bio Page
  • For walk-in applicants please submit the requirements at the Registrar’s Office 
  • Note: All school documents should be with English translation and duly Apostillized by the Ministry of Foreign Affairs located in the student applicant’s country of origin or legal residence or one (1) original copy of School Permanent Record in English duly signed by the Principal/Registrar of the school.  The original record should be placed in a sealed envelope with the signature of the Principal/Registrar on the flap of the envelope.
  • SPECIAL STUDY PERMIT – For Kindergarten, Elementary, High School Applicants and for Foreign Students below 18 years old 

Documents Required:

  1. General Application Form duly accomplished and notarized (BI Form No. MCL-07-01)
  2. Affidavit of Guardianship executed by the parents appointing a guarding for minor student
  3. Passport with valid stay and Visa
  4. 2 pieces recent 2×2 pictures white background
  5. Special Study Permit fee 
  6. Tourist Visa Extension Fee

NOTE:

  1. All requirements must be presented to the Registrar for verification and evaluation before an applicant can be issued a Notice of Acceptance.
  2. Incomplete requirements will not be accepted.
  3. Special Study Permit/Student Visa will be required before official enrolment.
  4. Applicants are required to follow the schedule of processing to avoid delay.

STUDENT VISA EXTENSION

  1. Application for an extension should be filed 1 month before the student visa expires.
  2. In case of failure, dropped or left, the student will be required a certification of re-admission from the Dean and a Letter of Explanation addressed to the Chief of Student Desk, Bureau of   Immigration.
  3. Student submits all the requirements for Student Visa Extension and pays for all required processing fees. (Student Visa Extension Fees )

*Study Permit/Student Visa fees may change as the need arises.

MODE OF PAYMENTS

A. Payment can be made directly to the school through the Finance Office

B . You may also request online banking payment. we will send you privately the bank account number. Just email us at pckhsaccounting20@yahoo.com

 If payment was done online or over the counter, please send a scanned copy of the receipt with the name and grade of the student to pchenkuang@gmail.com. Once payment has been confirmed, a copy of the OR and Registration Form will be sent to the email address specified in the copy of the receipt.
